Services Offered

Go to your home on a weekly/biweekly or monthly basis to fill Medi Sets and ensure compliance with medications.
Medication safety: education and monitoring of side effects with regular visits, possible or potential med to med interactions caused by your meds & how to reduce risk.
Will provide you or loved one with up to date Medication List with dosage and instructions to clarify plan prescribed by the provider.
Know that you or loved one is being cared for by an experienced & licensed RN that is able and qualified to perform this task.

Drive (advocate discretion), arrange for transportation or meet client at doctor's appointments and provide support during appointments.
Oversight of care and support system during hospital stay via regular communication with hospital staff, also able to provide hospital visits if within Sonoma, Napa or Marin counties.
Assist with adhering to discharge instructions during transition from hospital to home.
Assist with making doctors appointments and organizing medical care.
Communicate with providers to facilitate care.

Help the older adult age in place and stay at home as long as possible.
Experience with dementia care. Can make regular visits to ensure they are safe and have needs met and can communicate findings and suggestions to loved ones/team.
Provide client and team with critical resources to assist with care.
Communicate with ALL members of team, including providers, fiduciary, family, etc. Work very well with private home care agency caregivers to educate them and increase safety for client.
Provide vital support and education to caregivers.
